I've seen some users have their custom user title on multiple lines, how do you do this without HTML?

VIP ONly or you can set yours in your settings

VIP members are only allowed HTML usertitles. Donators can only customise their text, so if you do see a donator member with multiple lines then you can let a mod know.

What if a donator has just came from VIP, will there usertitle be changed?

Anniversary VIP from being on the forum for 5 years/6 years/and so on.
Other VIP and be obtained from winning competitions, and being in management, but can not be purchased like Donator - with the exception of Charity VIP which is sold around Christmas :).
